Registration Form
The registration price is $645 and includes all meals.
All boxes are required to process the form. If you encounter problems with the form and/or the payment, please contact us via email at
webmaster@badgeoflifecanada.org.
Once your payment is processed, you will receive a confirmation email. In the email, click on the “View Order Details” to go to the dashboard for the order. At the bottom of the dashboard, click on the “Download Receipt/Invoice” box to get a copy of your invoice that you can then print. Disregard the “Dashboard” page that appears after your payment, use the link in the confirmation e-mail only.
If you wish to pay by cheque, complete the form below, print it, and mail it with your cheque (made payable to “Badge of Life Canada”) to:

Please note that lodging is not included in the registration fee. You are responsible for your lodging. We recommend Horseshoe Valley Resort so you don’t have to travel to and from the Event. Please click on the link below for additional information:
